For melasma that does not respond to over-the-counter hydroquinone creams coupled with excellent UV protection, try betamethasone valerate 0.1% cream mixed 1:1 with tretinoin 0.025% cream plus 6% hydroquinone powder plus 1% ascorbic acid. This is applied sparingly at night to areas of melasma or other hyperpigmentation. Works like a charm.  ■
