    A 54-year-old woman presented to an outpatient office for recurrent erythema and tenderness of the right lower eyelid and cheek (Figure 1).

    Patient history. She was not taking any medications, and her health was excellent. She denied having a fever or drainage from the site. She had no history of diabetes or thyroid abnormalities but reported intermittent episodes of facial swelling and erythema occurring about once per month, each lasting about 2 to 5 days. Her history was remarkable for numerous malar and tear trough fillers over the previous 5 years.

    Diagnostic testing. A computed tomography (CT) scan of her head and sinuses was unremarkable. Laboratory studies, including an antinuclear antibodies test and routine chemistries, were within normal limits. The area was tender to palpation but not fluctuant.
